吉训明,男,1970年12月生,中共党员;神经外科专家,中国工程院院士、中医医学科学院学部委员,国家卫生健康委百万减残工程专委会主任,首都医科大学副校长、北京脑重大疾病研究院院长、首都医科大学宣武医院卒中中心主任;主要从事脑卒中防治与转化医学研究。

主要成就
专业特长及学术成就
专注于我国动脉和静脉性卒中发病机制、脑血流重建与神经保护研究。发现急性脑缺血的线粒体转移保护新机制,提出血管与神经联合保护新理论,创建缺血适应防治脑卒中、常压高氧(NBO)联合血管再通救治脑卒中和介入靶向低温脑保护等卒中续贯救治新技术体系。同时,在国际上率先创建了脑静脉性卒中的多模式救治体系,并研发专用治疗器械用于临床,成果写入国际指南和国际经典教科书。
针对脑卒中神经保护治疗临床转化不理想的困境,研究并提出脑缺血后“血管内皮应激-神经元求救-胶质细胞线粒体能量转移”的卒中脑损伤新理论和“血管优先”的血管神经联合保护新策略;创立卒中靶向低温救治新技术,研发自体血低温器官保护设备,解决了低温治疗卒中的转化难题,成果发表于自然(Nature)、科学通报(Science Bulletin)、自然通讯(Nature Communications)等,被国际誉为“脑卒中治疗的里程碑”。
其次,在疑难与重症卒中救治方面取得多个重大突破。提出重残率和死亡率超过80%的脑干血栓救治新策略,使重残率降低33%,死亡率降低27%,成果发表于新英格兰医学杂志(N Engl J Med);针对极易误诊误治、重症病死率高达30%的脑静脉血栓,建立生物影像诊断新技术和多模式救治新策略,使早期确诊率提高约3倍、救治成功率提高约1倍、病死率降低2倍以上,成果写入国内外指南和国际教材(美国神经外科教材第八版《尤曼斯-温神经外科学》和美国急诊医学教科书第九版《罗森急诊医学》等)。
第三,针对我国脑卒中以颅内动脉粥样硬化狭窄为主要病因、预防效果不佳的困境,提出束臂-脑缺血预适应新理论,研发专用治疗仪器和临床防治方案,是高危卒中年复发率降低27%,心脑血管意外发生率降低30%,成为预防脑卒中发生与复发的有效干预新手段,破解了我国特有的卒中防治难题,成果发表于高血压(Hypertension)、卒中(Stroke)、循环(Circulation)和柳叶刀神经(Lancet Neurology)等,成果入选2017年国际卒中领域重大进展。技术拓展用于高原病防治,组建北京高原适应研究康复中心。
